Welcome to Blyths Meadow Surgery
Semi-closure of the practice - 10 December 2021
The practice again is having to implement a semi-closure of its front doors due to patients attending the practice who have had a confirmed COVID test. All patients will be triaged as usual and video consultations will take place, however once triaged if the Doctor, Nurse etc wishes to see you in person they will make the arrangements for you. You will have your temperature checked prior to entering the building for your appointment.
All other visitors to the practice will need to use the side window of the building as we have done previously.
We apologies for having to take this action but we need to ensure the safety of all staff so that we can remain here to support your needs going forward.
Please have patience with all staff members during these difficult times for the surgery and you as patients, we will get to you call either on the phone, at the window or front door as quickly as possible.
Wearing of Face Masks after the 19th July 2021: -
The surgery has taken the decision to ask patients to wear face masks when visiting the surgery for anything, unless you are exempt.
This will ensure our staff and other services users remain protected against the ever changing COVID situation and we can remain open as a surgery.
We thank you in advance for your co-operation.
Referral Support Service Contact details:
If you are calling about your referral to hospital to see a consultant, please call the Referral Support Service on Patient line 0300 123 0771 between 8am - 6pm Monday to Friday. This service is unable to to advise if you have been referred for a scan.
Demonstrating your COVID vaccine status: From 17 May 2021, people in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses of any approved vaccine. Vaccine status will be available from:
You will need to register to use the online services, if you have not already. It may take more than a week for your identity to be checked and verified.
If you cannot access the online services, and you have had both doses of the vaccine, you can request a paper letter from the NHS by calling 119. Only call 119 if you are due to travel outside the UK in the near future and have had your second dose more than 5 working days ago. It may take up to 7 working days for the letter to arrive.
This practice is not able to provide you with a letter that shows your COVID-19 vaccination status. Please do not contact the practice about your COVID-19 vaccination status unless you have been advised to by the 119 service.
When you're planning your travel, you should check the latest information on demonstrating your COVID-19 status when travelling abroad on the gov.uk website. Make sure there is enough time to get proof of your COVID-19 vaccination status before you are due to travel.
Non Attendance of Appointments
The practice has to deal with missed appointment every week due to patients not giving us notice that they would not be attending. These "did not attends" (DNAs) are a waste of appointments that could be given to other patients.
Due to the increasing pressures faced by the practice this is not something that we can allow to continue. We will be monitoring those patients that do not attend on a regular basis. If you miss 3 appointments in a 12 month period, without a valid reason, you may be removed from the practice list and asked to register elsewhere.
For more information click here or the DNA Policy link in the menu to the right.
New Patient Registrations - the practice registration list is closed.
